Accidents happen where you least expect them -- but there are some lessons we have learned. At this years PMA in Antwerp, EEMUA chairman Dr. Andrew Lichnowski sums up the recent trends in process safety. Despite automated systems and elaborate alarm systems, process accidents continue to happen and are often contributed to the 'human factor', Dr. Andrew Lichnowski, chairman of the EEMUA Instrumentation and Control Comitee, explains.
Basic consequences and learnings of recent incidents are not only a better understanding of how automation and control systems and onsite workforce work together but also the need for a safety culture throughout the company.
